Assessing the Effect of Tumour Laterality on Lung Dose in Breast Cancer Radiotherapy

Objective: To explore the effects of laterality of breast cancer upon the irradiated lung dose.
Study Design: Retrospective observational study.
Place and Duration of Study: Tertiary care hospital Pakistan on patients treated between Jan to Jul 2023.
Methodology: The records of a total of 103 patients of breast cancers treated with curative intent radiotherapy were analysed. All had received an equivalent of 50 grays in 25 fractions or more of radiotherapy. The volume of lung irradiated in 95% isodose line was determined by central lung distance (CLD) and effect of laterality of tumour was seen on lung dose. The effect of presence of heart and its effect on potential reduction in lung dose in left sided breast cancer was determined and compared with the respective doses in right sided breast cancers.
Results: Among 103 patients, 57(45.3%) patients were having right sided breast cancer and 46(44.7%) were having left sided breast cancer. Mean CLD on right was 2.83±0.63 cm and on left side was 2.32±0.51 with a p-value of <0.001.
Conclusion: In appropriately planned Intensity modulated radiotherapy (IMRT) of breast, presence of heart leads to cautious planning on left side and decreased ipsilateral lung doses, as compared with right sided tumours, so if similar cautious approach is used in right sided breast IMRT, dose and toxicity of right lung be decreased.
