Abstract

Medical science has produced remarkable advances in diagnosis, treatment, surgical techniques, and patient care over the last few decades. Despite this progress, many patients still do not receive the full benefit of evidence-based clinical care. Treatments that perform well in clinical trials often produce different results when introduced into routine hospital practice. Limited resources, heavy patient load, variations in clinical practice, poor coordination among healthcare teams, and patient-related factors frequently reduce the effectiveness of proven interventions. This gap between scientific evidence and routine clinical practice has created a growing need for research approaches that not only generate evidence but also improve the way healthcare is delivered. Operations research and implementation research have emerged as two complementary approaches that can help address this challenge.

Operations research focuses on improving the efficiency, quality, and performance of healthcare systems by applying analytical and quantitative methods to clinical decision-making. It aims to answer questions such as how to reduce waiting times in outpatient departments, how to improve operating theatre utilization, how to optimize intensive care bed occupancy, or how to allocate limited healthcare resources without compromising patient care. Originally developed during the Second World War for military logistics, operations research has gradually become an important component of healthcare management because modern hospitals function as highly complex systems where every decision influences patient outcomes. 1
